Riders 18 & older Fill out Adult Release

Riders 17 & younger Parents/Guardian and rider must fill out BOTH of the Unlimited Sports Minor Release & Insurance Required Minor Release Forms. Insurance Minor Release Form MUST be Notarized and presented during registration unless membership was purchased online.

 **Please bring confirmation (email receipt) to registration at the track. Annual Release form is valid for 2024 Florida Gold Cup Series, 2024 Mini O’s, and 2025 Winter Am Series.
